If an individual requests disclosure of their personal information, the Company will promptly provide the disclosure.
However, if the identity of the individual cannot be confirmed, the Company will not respond to the request for disclosure. If there is an error in the personal information or if the individual requests disclosure of records related to third-party
provision, correction, addition, or deletion of the information, the Company will investigate and promptly address these requests.

We will send you a "Personal Information Disclosure Request Form," which you should
complete with the required details and return to us by mail or email. Please note that there is
a handling fee of 1,500 yen (tax included) for the disclosure process, payable at the time of
your request. The fee should be transferred to a designated account, and we will provide the
account details when you contact us.
① Method of Identity Verification for Requests from the Individual
We will verify your identity by phone. If verification by phone is not possible, please include
one of the following documents.
A copy of your driver’s license
A copy of your passport
A copy of your health insurance card
A copy of any other official document that can verify your identity
Please ensure that only the prefecture of your registered domicile is visible, and black out any information beyond that point.*
②Method of Identity Verification for Requests Made by a Representative
If you delegate the request for disclosure, etc., to a representative, please enclose the
following documents along with the Personal Information Disclosure Request Form
1) A document to verify the identity of the representative (copy).
Please enclose one of the following documents:
A copy of your driver’s license
A copy of your passport
A copy of your health insurance card
A copy of any other official document that can verify your identity
Please ensure that only the prefecture of your registered domicile is visible, and black out any information beyond that point.*
2) Power of Attorney
Please affix your seal on the power of attorney and attach the seal registration certificate.
※If the representative is a legal guardian, such as a parent or other statutory agent.
Instead of a power of attorney, you may submit a document that clarifies your relationship
with the individual, and that will be acceptable.
c) Method of Responding to Requests for Disclosure, etc.
We will respond using the method you specified at the time of your request for disclosure, etc.
However, if it is difficult to respond using the method you specified, we may consult with the
requester in advance and change the response method.
